Who qualifies
To apply for assistance you must have been employed for a minimum of six months with Commonwealth Senior Living, complete the application, and include all supporting documentation. If you have questions about whether your situation qualifies, contact us.
How it works

Three simple steps

Apply online

Complete the secure online application and tell us about your situation. The application is now fully online.

Upload documents

Take a photo with your phone or scan and upload your supporting documents. Include everything at the time of application to avoid delays.

3

Receive a decision

Applications are processed every Thursday. A determination response is typically sent the following Monday.

Note: the Selection Committee may request additional information, including monthly household expenses. If an applicant is unable or unwilling to provide such information, the application will be deemed incomplete and will not be considered.
